Brock Rondeau: Which is inability to sleep, snoring and sleep apnea 90 million people, that's almost afford to the population.
Mike Wiegenstein: That treatment options feedback, we don't ever really see and we'll science, don't let friends drive drunk. Brock Rondeau: Right.
Mike Wiegenstein: But yet we never see that depressed.
Brock Rondeau: If you sleep apnea, they will not give you the license to drive at motor vehicle.
Mike Wiegenstein: If you have severe sleep
Brock Rondeau: If you have severe sleep apnea you have to be under treatment.

Mike Wiegenstein: Welcome back, I'm in studio with Dr. Rondeau talking about sleep apnea and TMJ condition that effects close to 90 million people in North America. Alright doctor before went to break we're talking about sleep apnea children, sleep apnea adult I mean does it really have a particular person effects you told me --children is young as 4, 5 years old?
Brock Rondeau: That the -- Mike is prevention. If we can prevent these problems with the children and they'll never have sleep apnea and they'll never have TMJ problems and so that's why I like using these functional appliances to bring the jaws forward. When you bring the jaw forward, you bring forward open the air way.
Mike Wiegenstein: I want to tell you when you with I first only then I would bring the jaw forward. I'm think that I did research there are doctors that do surgery to -- and move the jaw forward it's sounds like it is a very involved painful process, but you are telling me there is no basically no pain at all.
Brock Rondeau: No pain at all, you were adrenal appliances like two retainers that bring the jaw forward it's very, very easy.
Mike Wiegenstein: Now they wear those at night when they sleep.
Brock Rondeau: Wear those nights, when they sleep.
Mike Wiegenstein: Okay, Now show me again what, what it looks like, because I don't think people that this is for an adult.
Brock Rondeau: First of all there is about 90 different designs, but certain -- designs but that they like the best but this particular design it's very simple and you can see it's a little hook hanging -- hook under here and that moves the jaw forward and the jaw can fall back at night.
Mike Wiegenstein: And if the jaw can fall back the tongue can fall back which keeps the air way open.
Brock Rondeau: Absolutely, it's very simple principle.
Mike Wiegenstein: Alright, now show me the one you did earlier for the children, that most of kids going this twin block
Brock Rondeau: This a twin block appliances that also keeps the jaw forward, it's got two blocks the inner lock at 70 degrees,
Mike Wiegenstein: Now can they open their mouth, then close you mouth, when it looks kids obviously to mouth less start closely -probably low cost to forward.
Brock Rondeau: I'm look in close but the only close once back, if they put this block and cover that block.
Mike Wiegenstein: It just makes it - forward
Brock Rondeau: They can close, so they always bring the jaw forward close like that and they do that. They wear that appliances for seven months, the jaw stays forward, tongues were stays forward. They never have sleep apnea, but now the only other thing with children have to make sure that there is no large anginoid tonsils get those tonsils manuals--
Mike Wiegenstein: Now, but if you went to the, they came to you with mom and dad, and they brought their child and you saw that you would then refer them how to have the tonsils remove first
Brock Rondeau: That's correct, I'm we work with in ear, and nose throat specialists to do that.
Mike Wiegenstein: Okay, the reason I really want to bring on A was because obviously the early prevention. But as you become an adult and up until recently obviously you told me all appliances has been around they've not been used. It's my understanding that if you were diagnosed with sleep apnea by most medical physicians went to at sleep by a rigid thing from mild sleep apnea to severe sleep apnea you ended up with this which is a CPAP right across
Brock Rondeau: That's correct.
Mike Wiegenstein: positive air way pressure, which basically is that we can reverse back in plan set a sucking any just constantly blows, I mean if you look at this and I want to talk you about numbers you want , you look you -- toes you impaired I mean if you rolled over your significant either your husband or your wife this is not the most romantic thing in the world and I can't imagine it's very comfortable to sleep in and you always have pressure blowing down your nose. People that have this get it needed wear, I have friends having they don't wear them. How many people after a year probably still using this?
Brock Rondeau: Probably 80% of people better given that don't wear after a year. The only once work I don't want to you really severe when they're say 350 pounds and they have got big necks and they absolutely have to have the air way opened up with that by blowing air up in nose. It blows out way the tongue and opens up the air way those patients need it
Mike Wiegenstein: Do you think the people that don't go, I am going to tell you I'm one of them you know one reason I want to talk when I is that the use of the appliance. That you told me I probably have sleep apnea some degree, I do, I have big neck but I'm not wearing that and I don't think I have a severe case. I mean obviously I could be okay with in appliance, but why do you think a lot of people may 9o million peoples or lot of few , why do you think they don't go to check, do you think it's there afraid to going to get the CPAP machine?
Brock Rondeau: Yes, I think it's a matter of education to lot of patients don't know their oral appliance were work. There is no body really advertising that oral appliances will stop more than sleep apnea that's one of the reason, I want to come on the program to help to educate patients, let them know that something can be done other than that. Most of my patients come in with that and they say they can wear it. They want an alternative, they want to stop this snoring, they want to sleep apnea, they want to stop the snoring because -- very upset and actually using good habits is the wife derives the husband and kicking and screaming.
Mike Wiegenstein: Okay, juts to be fare to sometime an husband derive the wife again kicking and screaming?
Brock Rondeau: Once in while
Mike Wiegenstein: Okay.
Brock Rondeau: The may over after menopause
Mike Wiegenstein: Yes.
Brock Rondeau: They are fat sometimes goes to there neck and they run in problems are also --
Mike Wiegenstein: Real quick because you said woman with the next to conference of greater than
Brock Rondeau: 16 inches, this is acceptable to sleep apnea
Mike Wiegenstein: And a man 17 or more than?
Brock Rondeau: The seventeen and i can see here more than seventeen.
Mike Wiegenstein: I'm a little more than 17. Okay now I come in, I to get check to get started to see if an all appliance will help me because obviously you know everybody scare they want to make sure nothing -- you brought about a little appliances to show me that you get people to see if the oral appliance is going to help remaining --
Brock Rondeau: In sometimes will use a little temporary appliance.
Mike Wiegenstein: Okay.
Brock Rondeau: This is vary and expensive with the little temporary appliance in and see if that works.
Mike Wiegenstein: The temporary holds the jaw forward with just at little paddle that success?
Brock Rondeau: Yes, the jaws picks in front of little --
Mike Wiegenstein: And then if you have success with that for those that are you won't worry then you move on into the other appliance.
Brock Rondeau: That's right, that's one thing we can do, but some patients want to go right to the permanent appliance, they don't want to go with temporary appliance.
Mike Wiegenstein: Now as an adult we've matured, we've grown can you still move the jaw I mean is it could you have success with that or there is something that's you know once our jaws and place it pretty there, we wear after were the appliance for the rest of our lives.
Brock Rondeau: No probably have to wear the appliance rest of our lives just a night.
Mike Wiegenstein: Just a night, but if you.
Brock Rondeau: When you go into bed just put it in and wake up morning, take it out.
Mike Wiegenstein: But if you get it as a child
Brock Rondeau: If you can prevent it another thing, I mean if you can get a child usually Mike it's the patient when the jaws that's far back. If you look at the profile to see that the lower jaw is back, if that's the case those patients needs functional appliances, when they younger, I start with 5, 6,7,8,9, 10 11.
Mike Wiegenstein: Can you literally move their jaw?
Brock Rondeau: Yeah, absolutely and it will stay there. If you do when they are actually gone
Mike Wiegenstein: Could you see on that they come after the nose you know your lips suppose be kind flat, which you see that chin it looks like, it's gone away, you can literally make cause that me before within appliance
Brock Rondeau: Absolutely, I'm going to show you some pictures after that you won't believe, patients that I have treated.
