Well, many people hook the live shrimp underneath the horn. They have a lot of trouble of avoiding the dark spots on a lively shrimp like this one.

It takes a little bit of care and a lot of people can't quite get it right and they kill the shrimp in the process. Now an alternative way, will be to hook the shrimp through through the body, the tail. Move from the underside out, through the middle part of the shrimp just like that and he will kick and stay alive to many cast.
