Host: What would be in the history that would make you have some concern that kid could have this thing we call WPW or some other related heart condition? What would you be looking for in the history?
Andrew D. Blaufox: So if patients -- depending on what exactly the condition is the symptoms with which patients present may vary a little bit. Patients with SVT or WPW might suspect that they would have palpitations or the sensation that their heart is beating rapidly for a long period of time and then when these episodes stop, they stop suddenly in just one week and that would be highly suspicious for someone having SVT.
Other dangerous electrical conditions such as Long QT syndrome, Brugada syndrome are they typically present with symptoms such as syncope or passing out or and the way that these patients pass out is atypical compared to patients who pass out for more of the nine reasons then they pass out during exercise and very suddenly and without.
Host: So it keep quite if you were big, if he was playing sport and they had attack of cardiac more than they expected or they had just pain while playing sport not talking not playing sport, that’s a little bit of red flag?
Andrew D. Blaufox: If the symptoms during exercise, if you have syncope during exercise then that is a red flag. If you are having palpitations during exercise that may or may not be.
Host: It’s kids to say to better than absolute.
Andrew D. Blaufox: It's set something that you would have to consider, but I did really I think that if the child is old enough and you can get a good history, if you can tease out the nature of these palpitations and well enough they seem to well or not regardless of whether or not the patient is exercising, it could be inductive of the SVT.
Host: Most of time chest pain in kids is usually not cardiac but it is involving an exercise, we always considered a big red flag when in the child a little bit more, is that true?
Andrew D. Blaufox: Yeah, chest pain in particular you have to be a little bit more suspicious, it’s occurring during exercise. I think not all chest pain during exercise is going to have a cardiac ideology, but you have to I think inquire a little bit.
Host: We are not saying it is but we are saying that it’s a red flag, a concern which should be evaluated.
Andrew D. Blaufox: Right. So then things that question and to keep in mind is whether or not the nature of the chest pain, if it sharper or more of a pressure if their associated symptoms and if there are things that can exacerbate the chest pain such as moving or deep breathing. But if it's not clear that this is a muscle or skeletal issue whether or not it's occurring with exercise then it should be evaluated by a cardiologist.
