Dr. Michael Franchetti: The most important thing is for the patient to be honest and give a complete history in as much detail as possible.
You're here today for an evaluation of the injuries that you sustained on the 15th of October of last year. I am asked to express the whole gamut of opinions. Tell me a little bit about the injuries you sustained, whether the accident caused these injuries.
Patient: Actually, there was a car in front of me that was completely stopped and I stopped right after them, but the woman behind us didn't and plowed into the back of me.
Dr. Michael Franchetti: Where was your first medical attention?
Patient: Our family doctor, Dr. Rastogi (ph).
Dr. Michael Franchetti: What areas were injured?
Patient: It was my neck and my lower back.
Dr. Michael Franchetti: Okay. Did he take any X-rays?
Patient: We just did a few to see if there was any bruising. He did prescribe me a pain reliever.
Dr. Michael Franchetti: What were the medications, do you recall?
Patient: No.
Dr. Michael Franchetti: I can look them up. Okay, Ibuprofen and he gave you some Flexeril as well. Did you have any physical therapy?
Patient: No.
Dr. Michael Franchetti: Are you still having any residual problems? Does this patient who still has these findings and complaints, do they need any more evaluation and or treatment?
Patient: Most definitely yeah. When I turn my neck to the right, it's mostly so very painful and my lower back is so very irritated.
Dr. Michael Franchetti: What usually brings on the neck pain?
Patient: I work at a computer all day, sitting up and turning my neck staring at the computer, irritates neck and back.
Dr. Michael Franchetti: What kind of work you do?
Patient: I'm a Title Processor.
Dr. Michael Franchetti: Did you miss any work because of this injury?
Patient: No, I didn't.
Dr. Michael Franchetti: Okay. Any sports or hobbies or leisure activities that are limited or affected by these injuries right now?
Patient: Well, usually when I go out with my friends and dancing, it just becomes so irritated that I don't even want to anymore. So it's hard for me too.
Dr. Michael Franchetti: Your dancing is limited and painful?
Patient: Very much so.
Dr. Michael Franchetti: Sure. Also, a very important question, in the past, have you ever had any injuries or symptoms to your neck or back?
I'm trying to find out if some of her complaints are due to this injury or possibly a previous injury you're finishing.
Patient: No.
Dr. Michael Franchetti: I would like to take a look at you and go through the examination, okay.
Patient: Okay.
