Dr. Simon Hall: This is after a prostate removed, what will the ejaculation be like?

Dr. David B. Samadi: Well, the ejaculation will not be there and a lot of times the patients will tell me that they do ejaculate after this. I think we can talk about the Cowper's gland and some of the secretions from the peripheral glands that may be causing some of the secretions. But the bulk of ejaculation, which is part of the seminal vesicle is removed, while the orgasm you will be able to have a 100 percent orgasm because that's through the nervous system. The ejaculation will not be there and with the kind of nerve sparing that we have seen here, we see especially the young patients in their 40s and 50s are able to obtain their sexual function and you have heard this before, but a lot of times even with the catheter, they will have the erections.

So, really the take home message is stay away from the lateral tissues. The less you do, the better. This is certainly robotic prostatectomy, or open prostatectomy, may not be for everyone and unless you are committed to this and unless you are willing to spend the time and go through the learning curve and the volume, this is going to be a little challenging.
