Teams Approach - Cleft Palate / Lips

Interviewer: If you are going to use the dentist with complexities, you probably want to be very familiar with this and a big sea like this, we have a lot of pediatric dentists around probably, but is trained for this and the general dentist, I should have some general dentist that got training in this particular area of the children, is that true? Doctor: It is true, but you really wanted dentists that works with the cleft team. Cleft lip and the treatment of kids with the cleft lip and cleft palate is a whole arts and science… Interviewer: So it will be a dentist that has special additional training? Doctor: You wanted someone who works with the cleft team. A children’s dentist who not only treats kids, but also works with a team… Interviewer: But he got a more specialized pediatric examples. Doctor: Exactly. Interviewer: Kids have obviously with teeth coming in, on occasion, teeth spacing of teeth that may not be lined up correctly. Doctor: One of the problems is that kids that fall outside the team care, they do not necessarily get a thorough care because they get to someone who may not be familiar with certain techniques that are used, anticipates certain problems and even in your attempt to try to help the kids, they are not as well informed. People that stay with the team, doctors and therapists that stay with the team really are in the cutting edge and know just when and how to do things. The concept with the cleft team is that each procedure is going to be filled and get ready for the next procedure. Interviewer: If you had a kid born with a cleft lip, palate or combination, today you can expect great results in most of the cases especially if you use the team approach. It does not look so good. You could see from some of the pictures that I have seen around here that you have start of with a problem, you hardly will notice unless you agree of course you see a little wine or something. So it is a great resolve of course and result is going to depend on the complexity of all the other problems, is that true? Doctor: It is true. It does start out with a fairly frightening problem and I want to mention this that over the past three years, things have changed a bit. Probably when I started practicing and got involved with interesting cleft lip and cleft palate care, it was the therapy with a total surprise who learned that the baby was born unless, of course the parent had a cleft lip or they had another kid, they will not suspect it, but right now, all pregnant mothers have ultrasound. The ultrasound is so sophisticated that they can visualized a cleft really at the three month interval, at the three-month stage of pregnancy. And so they can now tell the parent, they think there is going to be baby born with a cleft lip and they can follow it, manage as the baby gets older and they can see it. A parent is well informed and does not have that initial shock. Interviewer: Also you can check out the team you want to use with such problems, right? Doctor: Right, a lot of times I see a parent and comes to me beforehand and I will be able to explain things and let them know. That is a big benefit, I think. It just takes the shock a way they are long informed and they are ready for it. It is a fragile period of time the baby is being born and you want it to be really enjoyable period of time and you do not want to shock them with something that will… Interviewer: In other words, it provides major other problems and it is only the defects that revolved, you expect great results down the road? Doctor: Right. Interviewer: What if that fits the current results down the road? --
