Interviewer: Sometimes you see a kid and out of nowhere, there seem to be like earrings of the skin and the parents are worried the kid is going to lose all the pigment, what is that all about?
Interviewee: We find that a little alike oh, rarely affects in the family once in woman, however, it is an autoimmune disease which is a fancy word meaning for who the hell knows what causes it because it is like juvenile rheumatoid arthritis, lupus, any of these things. Now, it is mild, it is cosmetic, however, depends what ethnicity you are. So, if you are in the impact standing, black, it is a whole lot of issue, other reason you know, because now, that makes a big deal. And it makes a big deal for other reasons. Interviewer: Is there any treatment that you could do that are successful?
Interviewee: Yes, original treatment was approved but I think that has fallen out of favor because it is a systemic drug and there is a whole this little problem. (1) It is a drug, need to take it by mouth, you connect the eyes for 24 hours afterward because it stays in the system for 24 hours. It increases your chances of skin cancer. At least in psoriatics. And you are looking at two twice a week for a year under treatment. The bitten, so down from that, now, as now a band you will be big which is an ultraviolet, no drug. It works four months as well but it still takes a lottery in this. And the last thing which works better than our band UV be but it cannot do a whole body, it is laser. So it is called an X summer laser, it is in the same wave length, it is 308 so it is an ultraviolet, it takes the peak and concentrate the light into a fiber so you are thanking the power of all this sunlamps and come up with a spot so now you choose it up and that works in as little as 15 visits. The best places are face, neck, chest is okay, back, tough areas, really really tough areas are fingers and toes.
Interviewer: They want to be treated or it gets better by itself.
Interviewee: It might get better by itself but I cannot tell that because I do not see people get better on themselves and some people, it just keeps going.
Interviewer: Is there any diseases that does not like this?
Interviewee: You are supposed to check autoimmune diseases so you are suppose to check for thyroid problems because there is slight incidence of it. I have never seen the case of lupus that crosses it but basically looking for thyroid. In the old days, probably 40 years ago, you are suppose to look for pernicious anemia with it because that was an autoimmune disease in the old days, so you are suppose to look for that. It is a question of how cosmetic leaves figure in this so obviously black person face, is cosmetically disfigured. So it would be nice to treat them earlier if it is going to psychologically bother them. You do not want them to get to where they are missing 40 to 50% of the color, it is, and you lost. It is when you are missing more color than you have; you are never getting that color back. You have to stimulate the cells that are still there to grow back. Interviewer: Are there any never to giving steroids to this people?
Interviewee: Systemically, I would not trade it off because you are not getting off too fast. Topically, I do not think it does much. Protopic which is a calcium Neurontin inhibitor which is a non steroid, does something by itself. Vitamins work in South America very well except they do not work as well here. So, South Americans uses mega doses of vitamins than guns. Some improvement before the laser came along; I used to use mega doses of vitamins.
